چکیده

Background Atrial fibrillation (AF) is the most common arrhythmia after cardiac surgery, yet precise incidence and significance of arrhythmias discharge home need to be better defined. Photoplethysmography (PPG)-based smartphone apps are promising tools enable early detection follow-up arrhythmias. Objective By using a PPG-based app, we aimed gain more insight into prevalence AF other rhythm-related complications upon surgery evaluate implementation this app routine clinical care. Methods In prospective, single-center trial, patients recovering from were asked register their heart rhythm 3 times daily Food Drug Administration–approved for either 30 or 60 days home. Patients with permanent pacemaker excluded. Results We included 24 (mean age 60.2 years, SD 12 years; 15/23, 65% male) who underwent coronary artery bypass grafting and/or valve surgery. During hospitalization, 39% (9/23) experienced postoperative AF. After discharge, PPG reported atrial flutter in 5 patients. While notified 1 patient, was false positive, as electrocardiogram revealed 2nd-degree, 2:1 atrioventricular block necessitating pacemaker. confirmed 4 (4/23, 17%) interestingly, associated an underlying complication 2 participants (pneumonia n=1, pericardial tamponade n=1). A significant increase proportion measurements indicating sinus observed when comparing first second month (P<.001). follow-up, compliance significantly lower 2.2 (SD 0.7) per day versus 3.0 0.8) (P=.002). The majority (17/23, 74%), well surveyed primary care physicians, positive value by they felt involved rehabilitation. Conclusions Implementation smartphone-based technology enables An association between found Therefore, may supplement rehabilitation acting sentinel complications, otherwise.

Active Bleeding after Cardiac Surgery: A Prospective Observational Multicenter Study

MAIN OBJECTIVES To estimate the incidence of active bleeding after cardiac surgery (AB) based on a definition directly related on blood flow from chest drainage; to describe the AB characteristics and its management; to identify factors of postoperative complications. METHODS AB was defined as a blood loss > 1.5 ml/kg/h for 6 consecutive hours within the first 24 hours or in case of reoperati...

Introducing intravascular microdialysis for continuous lactate monitoring in patients undergoing cardiac surgery: a prospective observational study

INTRODUCTION Lactate is a marker of hypoperfusion and may be used for risk assessment in critically ill patients. Although evidence suggests that repeated lactate measurements are of clinical interest, how and when lactate should be analyzed is controversial. Intravascular microdialysis provides a novel method for the continuous monitoring of lactate, which may be clinically beneficial in criti...

Incidence of chest tube clogging after cardiac surgery: a single-centre prospective observational study.

OBJECTIVES Chest drainage following cardiac surgery is used to avoid complications related to the accumulation of blood and serous fluid in the chest. We aimed to determine the incidence of chest tube clogging and the role of bedside assessment in identifying the potential for failure to drain. METHODS Data from 150 patients undergoing cardiac surgery using cardiopulmonary bypass from March t...

A novel treatment strategy of new onset atrial fibrillation after cardiac surgery: an observational prospective study

OBJECTIVE The aim of this prospective observational study was to evaluate the efficiency of a new escalating treatment strategy with vernakalant, flecainide and electrical cardioversion (EC) in patients with new onset atrial fibrillation (AF) after cardiac surgery. MATERIAL AND METHODS 24 patients with new onset AF after aortic valve surgery, coronary artery bypass surgery or combined procedu...

Comparison of direct and indirect blood pressure in patients under esophagectomy surgery: a prospective observational study

Background: Arterial pressure is one of the most important physiological variables and often needs to be monitored repeatedly or continuously in perioperative period. Arterial pressure monitoring is one of the standard monitoring in operating room. During general anesthesia, blood pressure can be measured by using a noninvasive arterial pressure method or continuous invasive arterial pressure b...
